Make a difference through compassionate, coordinated care Are you an organised, compassionate and proactive healthcare professional looking for a rewarding role where you can make a real difference to patients and their families?
We are looking for an experienced Care Co-ordinator to join a specialist palliative care service supporting patients and families across the local community.
This is an important role within a 24-hour advice and support service, helping to ensure patients receive timely, compassionate and well-coordinated care. You will work closely with registered nurses and a wide range of healthcare and community professionals to coordinate referrals, support care pathways and help patients remain in their preferred place of care wherever possible.
You will also play an important role in supporting joined-up working and helping to prevent avoidable hospital admissions through effective communication and coordination between services.
About the role As a Care Co-ordinator, you will:
About you You will have experience of coordinating appointments, referrals or patient pathways, ideally within a healthcare or community setting. You will understand the principles of palliative care and the importance of providing holistic, person-centred support to patients and families.
