JOB TITLE: Care Coordinator – Domiciliary and Complex Care
REPORTS TO: Registered Manager / Deputy Manager

MAIN PURPOSE OF THE ROLE
To coordinate the planning, scheduling, and delivery of domiciliary and complex care services in accordance with the Health and Social Care Standards (Scotland) and Care Inspectorate regulations. The Care Coordinator will work closely with the management team to ensure care packages are delivered safely, effectively, and in a person-centred manner, meeting the needs and preferences of each service user. This role requires proactive communication with staff, service users, families, and other professionals to maintain continuity and quality of care, including oversight of specialist and complex care provision.
REQUIREMENTS FOR THE ROLE
· Experience in care coordination, rostering, or similar role within the health and social care sector.
· Knowledge of the Health and Social Care Standards (My Support, My Life) and Care Inspectorate requirements.
· Ability to plan and manage care schedules efficiently while ensuring service user needs are prioritised.
· Experience or willingness to train in understanding the requirements of specialist and complex care provision.
· Excellent organisational and communication skills.
MAIN DUTIES AND RESPONSIBILITIES
· Coordinate the allocation of care staff to service users, ensuring continuity of care and adherence to care plans.
· Manage rotas effectively, including sickness and emergency cover, to maintain service standards.
· Support the Registered Manager and Deputy Manager in ensuring all care delivery meets Care Inspectorate quality requirements.
· Ensure that service user care plans are accurate, up-to-date, and reflect their current needs and preferences.
· Maintain effective communication with service users, families, staff, and external professionals.
· Monitor the delivery of complex care tasks to ensure they are carried out by trained and competent staff.
· Ensure safeguarding concerns are escalated in accordance with the Adult Support and Protection (Scotland) Act 2007.
· Maintain accurate records in line with the Data Protection Act 2018 (UK GDPR) and Yeshua Healthcare’s confidentiality policy.
· Provide guidance and support to care staff in the field to maintain high standards of care delivery.
· Keep up to date with relevant legislation, Care Inspectorate guidance, and best practice developments.
· Contribute to staff training, inductions, and competency assessments where required.
· Promote Yeshua Healthcare’s values of respect, dignity, compassion, and person-centred care in all aspects of service delivery.
