Annual Salary: £24,375-26,325 depending on experience

Hours: 37.5 hours per week - Monday to Friday 0900-1700
Requirement covering on call on a rota basis (additional pay)
Requirement to cover care calls as per the needs of the business.

MAIN RESPONSIBILITIES OF A CARE COORDINATOR

• Assist with the implementation and set up of new packages, working closely with all involved in this process
• Oversee all aspects of clients care packages (updating care plans and risk assessments, completing monthly reviews etc).
• Build and maintain professional relationships with clients and carers based on trust, respect and honesty.
• Assist with delivering effective staff inductions including meet and greets and shadow shifts.
• Complete care staff supervisions, appraisals and team meetings.
• Ensure all shifts are filled and plan in advance for any annual leave.
• Report any incidents/accidents or concerns in a timely manner through our internal recording system.
• Participate in the out of hours on call rota (additional pay).
• Complete any other appropriate duties and responsibilities as requested by the Registered Manager
• Complete any outstanding care calls as required by the business ( additional pay outside office hours)

EXPERIENCE/SKILLS REQUIRED FOR THIS CARE COORDINATOR ROLE:

• Minimum of two years care experience in domiciliary care.
• Resilience
• Knowledge of CQC guidelines

• Desire to succeed/motivation
• Computer literacy -experience in use of Care planner and Office suite
