Responsibilities

JOB PURPOSE

Responsible for the safe management of day-to-day client’s care, quality and compliance.

THE ROLE

Support the Senior compliance manager/ Head of Care Operations to ensure our clients receive the best possible care, and support, delivered with respect and dignity.
Liaise with clients, families, social workers, GP’s, District Nurses, Physiotherapists etc as required and always in a professional manner.

MAIN RESPONSIBILITIES

Clients care and quality:
· Ensure all new client initial assessments, care plans and risk assessments that are assigned to you are completed prior to the care package starting and ensure they are person centered and bespoke.
· Ensure all required documents are uploaded to ACP in the documents section for each client.
· Ensure all ‘Home folders’ including QR codes are in place in clients’ homes prior to care commencing.
· Ensure all new clients 1- week call after start of package to review the quality of the service so far and address any concerns with the designated department.
· Ensure a 4-week Quality and review visit in person- ensure outcomes are recorded for all you are responsible for and address any concerns with the designated department.
· Ensure a 3-month quality and review telephone call- ensure outcomes are recorded for all clients you are responsible for and address any concerns with the designated department.
· Ensure the 6 monthly care plan review and quality assurance with clients and client’s families (Home visit) is completed for all designated clients and address any concerns with the designated department.
· Ensure task matrix generated from People planner is kept in date and overdue tasks are not present by completing all QA’s, reviews within the required timelines.
· Ensure all client tasks on ACP are up to date and dates updated on People Planner.
· Ensure each phone call is answered in a timely manner or respond within 1 hour to voice mail messages. Record concerns on People Plan and create an event, assign.
Client Compliance:
· Complete daily notes auditing and medication audits, In line with daily notes process.
· Ensure all concerns raised in daily notes/medication audits are dealt with immediately and an evidence trail of all actions is recorded as in line with daily notes process.
· Prioritize and respond in a timely manner to all incidents / accidents and near misses, ensure investigations are completed in line with UK Care Team Policy and escalated to manager/appropriate department and raised a ticket/add an event and assign.

· Involvement in monthly quality meeting and report

OTHER RESPONSIBILITIES:

· Maintain Health & Safety standards in accordance with UK Care Teams policies and procedures, including undertaking risk assessments for all care related tasks, environments and medications.
· Complete all mandatory and CPD training as required and/or requested by line manager, or any further development your line manager feels appropriate to your role.
· Establishing and maintaining positive relationships with clients, clients families and professionals.
· Ensuring up to date, appropriate and accurate record keeping is maintained.
· Maintain and understand your own responsibilities in respect of infection prevention and control
· Maintain and understand your responsibilities in respect of safeguarding adults and children.
· Other responsibilities may be requested of you by your line manager or Senior management team, as required in line with the needs of the business.
· Support On Call Rota and be required to work every 3rd weekend with 2 days off in the week
Job Type: Full-time
Pay: £26,000.00-£27,000.00 per year
