What You Should Know About the Care Coordination Specialist position: * This is a FT 40hr per week positions with the hours of 7am-5:30pm 4 days a week and an every 4th weekend rotation and 2 holidays a year rotation required.  * This is an on site position at our Care Coordinator Center located in Centerville OH and will be with all teams across OHI territories * We provide superior care and superior services to patients at their end of life journey. Only those who have a heart for hospice will succeed   Job Summary:  Utilizing exceptional customer service skills, coordinates clinical schedules according to patient’s plan of care. Key Responsibilities:  * Uses appropriate customer service skills to ensure effective communication  * Participate in meeting patient physical emotional, spiritual, bereavement, and other needs, and evaluates response to care provided to ensure best outcomes for patient/family. * Schedules and coordinates patient and staff visits in a timely manner * Participates in Continuous Quality Improvements activities to support the quality of Ohio’s Hospice services * Precepts new staff to support professional relationships with newly hired team members as required Care Coordination Specialist Qualifications:   * High School Diploma or GED * Two or more years' experience with scheduling/dispatching preferred * Computer skills sufficient to properly schedule and communicate effectively.   * Ability to drive during daytime, nighttime or inclement weather   * Valid Driver's License with Safe Driving Record   * State Minimum Automobile Insurance Coverage   Benefits & Perks: your health and happiness matters!
