POSITION SUMMARY We are seeking a Care Coordination Supervisor to oversee the day-to-day operations of assigned CCSW and CCSI Care Coordinators, ensuring timely and effective delivery of care coordination services. This role supports the CCSO Program Manager in maintaining compliance with federal and state requirements, as well as agency and funder benchmarks. The Care Coordination Supervisor provides leadership, guidance, and coaching to the Care Coordination team, ensuring the delivery of high-quality, client-centered care and efficient program performance. KEY RESPONSIBILITIES * Directly oversee CCSW and CCSI Care Coordinators to ensure proper workflow, documentation, and performance. * Develop program goals and expectations and guide Care Coordination staff in achieving them. * Maintain confidentiality and adhere to ethical conduct in accordance with agency policies and procedures. * Respond to internal and external customer needs by utilizing the AIDET model to deliver exceptional service and outcomes. * Utilize the electronic health record system (Athena) to document patient data and manage outcomes databases. * Complete all required reports and documentation accurately and in compliance with funder requirements. * Collaborate across departments and with community partners to enhance coordination of care. * Monitor and analyze benchmark data and program performance metrics. * Provide direct supervision, performance evaluations, and team meetings to support staff development. * Participate in multi-disciplinary staffing meetings to ensure coordinated, high-quality patient care. * Communicate and ensure the proper documentation of incidents, including the completion of Incident Reports as required. * Attend trainings, agency meetings, and Core Curriculum sessions as required. * Perform other duties as assigned. QUALIFICATIONS * Master’s degree in Social Work or Counseling required. * Minimum of 2 years of experience in mental health services and 2 years of supervisory experience required. * Proficiency in Microsoft Office applications and data entry. * Strong knowledge of community resources and experience working with culturally diverse populations. * Excellent communication, leadership, and organizational skills.
