Description

At Adada Care Services, we provide care and support to Adults and Young Adults within Cheshire. Due to the consistent growth and development of our organisation, we are looking for a Care Coordinator to join the team on a full-time basis in our office in Chester.

Responsibilities

THE ROLE:

As a Care Coordinator, you will provide and coordinate individualised care and resources to our service users, aiming to enable every individual to receive care within their own homes.
This role is highly dynamic and hence requires a flexible and calm approach and the ability to review and prioritise tasks consistently.
You will be working with several different stakeholders, such as District Nurses, Social Workers and service users’ families to ensure that high- quality and person-centred care is provided.
You will be required to assess as well as review service users’ needs to design a care service tailored to each individual.
You will be using care planning softwares as well as Microsoft Office throughout your daily routine. Therefore, you will be required to be IT literate and open to learn new software quickly.
You will be working closely with other Care Coordinators, the Registered Manager as well as Training Managers and administrative staff.
Our office culture is open and transparent, with a constant drive to improve and innovate our services.
You will work every third weekend from the office, and have 2 days off in the week.

THE FOLLOWING CRITERIA ARE ESSENTIAL FOR THIS ROLE:

  • Full UK driving licence & access to a vehicle
  • Previous experience as a Care Coordinator in a domiciliary care setting
  • Holding an NVQ/RQF Level 3 in Adult Health and Social Care (or equivalent)
  • Must be flexible and able to work alternative weekends
  • Computer literate with experience in the use of Microsoft Office
  • Ability to demonstrate a comprehensive knowledge of care standards in home care
  • Ability to apply the knowledge and expertise in the practical day to day tasks
  • Able to work under pressure and time constraints
  • A good working knowledge of health & safety regulations
  • Resilient with good communication and interpersonal skills
  • Enhanced DBS Certificate
  • Two employment references (including one from most recent employer)
