CARE COORDINATOR

Salary: £13.50 per hour
Milage: 45p per mile (must have your own car for this role)
Areas: This role covers Southend. You will be mainly based in the Southend Office.
You must have a UK driving license and access to your own car with business insurance.

KEY SKILLS:

  • Level 3 NVQ in Health and Social Care
  • Substantial experience of working within a care environment
  • Computer literate and a good eye for detail to ensure customer records are accurate and up to date
  • Proactive – be able to forge relationships with health and community practitioners as well as finding referrals
  • Networking and forging relationships to gain a full understanding of the Community and what offerings are available
  • Experience with dealing with sensitive data and understanding of Information Governance
  • Ability to interpret community care assessments into Service User plans
  • Demonstrable knowledge of the CQC and legal requirements for community-based services, specifically for Domiciliary services
  • Strong ICT skills with demonstrable experiencing of working with care planning systems and care assessment tools
  • Ability to undertake role-based risk assessments and all associated paperwork which reduces/mitigates risk

Responsibilities

ABOUT THE ROLE:

This role is to carry out person centred initial assessments, outcome focussed support planning and subsequent customer reviews in variety of settings, including customer homes, both in person and via the telephone. You will need to gain and maintain a comprehensive knowledge of available local community resources and pro-actively liaise with our integrated team of health and social care workers, as well as other external health professionals, to maximise the support available to the individual. The role holder will be the main point of contact for customer and families throughout the process. The role holder will also identify and request any appropriate equipment.

DUTIES INCLUDE:

  • Process, allocate and refer all new work efficiently.
  • Process, accept and amend existing care packages.
  • Assigning care workers to the clients, whilst working within the standards and requirements of the company.
  • Ensuring all shifts and working patterns meet the needs of the company, service users, partners and care workers.
  • Liaising with NHS, Local Authority commissioning teams as well as private clients to take new care packages.
  • Liaising with members of our management team and head office.
