We are working with an established and growing Home Care provider in Woking, Surrey, who is looking for a caring, organised and motivated individual to join their team as a Care Coordinator.This is a fantastic opportunity for someone who understands care from the front line and is ready to take on more responsibility, develop professionally and build a career beyond hands-on care.

If you've had some involvement with staff rotas, scheduling, coordinating visits or arranging cover using ACCESS PEOPLE PLANNER, PASS etc..., that's a great foundation — and you'll have the opportunity to build on this with training and support.

  • The company is experiencing an exciting period of growth, with genuine opportunities for career progression and development.What You'll Be DoingWorking alongside the Registered Care Manager, you'll help coordinate the day-to-day delivery of care, including:Assisting with preparing rotas and scheduling, including arranging cover when required
  • Helping to ensure the well being of both clients and care staff
  • Supporting safeguarding procedures
  • Building positive relationships with clients, families and care workers
  • Liaising with Social Workers, GP practices, NHS Discharge Teams and other professionals
  • Supporting the Registered Care Manager with administration and day-to-day coordination
  • Providing personal care in the community when required
  • Participating in a weekday & 1-in-4 weekend on-call rota
