Overview:
The Care Coordinator-Behavioral Health-AOT provides professional services to patients in the Behavioral Health department to meet identified psychosocial, emotional, financial and environmental needs. The coordinator provides psychosocial assessments, supportive counseling, emergent crisis intervention appropriate to setting, financial resource information, environment enhancements, advance directive planning and referrals to community agencies for patients. This position will deliver services outlined in grant funded programs within the department, including, but not limited to, participants in the county’s Assisted Outpatient Treatment (AOT) Program. Using an interdisciplinary team approach, the coordinator identifies and implements interventions at the individual and systemic levels and provides expertise to high-risk clients.

Responsibilities:
Provides social work intervention to patient/families served by the department. Finding resources to assist the patient with: transportation, obtaining medications, financial barriers, social security or employee-based disability claims, or other barriers to safe and successful treatment. Collaborates with behavioral health team to meet the needs of high risk populations.Helps to facilitate care transitions and other specialties when needed.Documents all patient interactions, significant observations, interventions, and actions taken in the patient’s medical record in an appropriate and timely manner.Develops and maintains community relationships to support client referrals.Consults and collaborates with other professionals involved in providing services to patients. Assist patients with completing applications for assistance programs, including Illinois Medicaid, Carle Financial Assistance
Program, and pharmaceutical assistance programs. Attends client-related meetings in or out of the office including with psychiatric and medical prescribers as necessary. Documents and provides data entry for grant reporting and tracking for services delivered through grant funded programs Keeps the director/manager informed of problematic cases, especially those involving legal or risk management issues. Performs other duties as assigned. Delivers grant funded case work services for department grants, including but not limited to SAMSHA Assisted Outpatient Treatment. Assists with prior authorizations for provider services and medication management Coordinates services with hospital, outpatient service providers, law enforcement, and the court for individuals served in the Assisted Outpatient Program and other grant funded programs.
Qualifications:
Certifications: Licensed Social Worker (LSW) within 1 year - Illinois Department of Financial and Professional Regulation (IDFPR)
Education: Bachelor’s Degree or Master’s Degree
Work Experience: Ability to problem-solve and develop solutions for patients and their families, strong knowledge of available community resources, and awareness of when and how to access additional resources to meet a patient’s needs. Strong advocacy skills necessary.
