We are seeking an experienced Care Coordinator who is motivated, organised, and committed to delivering exceptional care and service to our clients.

REQUIREMENTS

  • Proven experience as a Care Coordinator in Home Care, NDIS, or Aged Care.
  • Strong knowledge of NDIS Practice Standards and relevant legislation.
  • Excellent communication and interpersonal skills.
  • Ability to manage competing priorities and meet deadlines.
  • Competent in Microsoft Office and client management systems (e.g., PRODA, My Aged Care portal).
  • Current Driver’s Licence and reliable vehicle.
  • Current National Police Check, Working with Children Check, and NDIS Worker Screening Clearance.
  • Relevant qualifications in Nursing, Community Services, or Disability/Aged Care Experience
Responsibilities
  • Coordinate and manage services for NDIS and Aged Care clients, ensuring compliance with individual care plans and funding requirements.
  • Conduct assessments, prepare service agreements, and develop care plans in consultation with clients and families.
  • Monitor service delivery to ensure high quality, safety, and satisfaction.
  • Liaise with support workers, allied health professionals, and external stakeholders to meet client needs.
  • Maintain accurate documentation and compliance with NDIS Practice Standards and Aged Care Quality Standards.
  • Oversee rostering to ensure timely and efficient service provision.
