Job Description: Job Title: Care Coordinator Location: Kirksville, MO Employment Type: Full-time Shift: Monday – Friday, 8:00 A.M. – 5:00 P.M. Join our team at Preferred Family Healthcare as a Care Coordinator and help make a meaningful impact in the lives of our clients. As a Care Coordinator, you will provide comprehensive coordination of services for individuals receiving outpatient treatment. You’ll work closely with clients, families, and providers to ensure continuity of care and access to essential resources. Position Perks & Benefits 29 Days of PTO Eligibility for HRSA Loan Repayment (subject to requirements) Comprehensive employee benefits: health, dental, vision, retirement, life insurance Competitive 401(k) Retirement Savings Plan – up to 5% match Company-paid basic life insurance Emergency Medical Leave Program Flexible Spending Accounts – healthcare and dependent care Health & Wellness Program Employee Assistance Program (EAP) Employee Discount Program Mileage Reimbursement Key Responsibilities Review initial assessments and discharge planning to identify client needs Coordinate outpatient services for mental health and/or substance use disorders Maintain communication with referral sources and primary care providers Request and manage correspondence from past treatment providers Participate in staff meetings to ensure continuity of care Facilitate referrals for services outside the scope of treatment Assist with scheduling and transportation arrangements Develop and update individualized treatment plans Advocate for clients and link families to community resources Support transitional planning for clients and families Address chart deficiencies promptly Document services in compliance with KDHE/JJA standards Provide crisis intervention as needed Meet weekly direct service expectations Complete 32 hours of professional development training every two years Education and Experience Qualifications Bachelor’s degree in a human services field (e.g., Social Work, Psychology, Counseling, Criminal Justice, Nursing, Education) preferred Required Licenses/Certifications Valid driver’s license and current auto insurance TB test (annually for direct care staff) Background check including criminal record, driving record, abuse/neglect, and fingerprinting First Aid and CPR certification within 3 months of employment Behavioral Management training (CPI or Verbal De-escalation depending on program) Completion of “Direct Service Expectations” within 30 days of employment Missouri Staff: 36 hours of training within 2 years (per 9 CSR 10-7.110 E 1) Additional Qualifications Knowledge of case management and rehabilitation methods Understanding of community resources and legal procedures related to mental health and substance use Ability to communicate effectively both verbally and in writing Capacity to evaluate services and contribute to program improvement Interest in both short-term and long-term organizational goals Brightli is on a Mission: A mission to improve client care, reduce the financial burden of community mental health centers by sharing resources, a mission to have a larger voice in advocacy to increase access to mental health and substance user care in our communities, and a mission to evolve the behavioral health industry to better meet the needs of our clients.
