We are looking for a Care Coordinator to provide leadership to our team members supporting individuals with mental and/or physical limitations in their homes.
This dynamic role oversees staff across Ionia, Montcalm, & Mecosta Counties and is crucial to ensuring the highest quality of care for the individuals and families we serve.

• Lead, evaluate, and train in-home care staff
• Ensure compliance with all training and care standards
• Conduct regular quality checks and home visits
• Oversee staff scheduling and business operations for your caseload
• Communicate effectively with CHS admin, guardians, and partner agencies
• Maintain documentation and confidentiality at all times
• Help build a motivated, caring, and reliable team
