We are an expanding GP surgery in Horsham looking to recruit a Care Coordinator to join our exceptionally friendly and hard-working practice team.
As a Care Coordinator you will work closely with GPs, clinical staff and the practice team to manage a caseload of patients, acting as a central point of contact to ensure appropriate support is made available to them and their carers; supporting them to understand and manage their conditions and ensuring their changing needs are addressed.
In addition to practice team you will also work with the wider multi-disciplinary team within the locality. You will be required to have an active role in appropriate meetings as well as organisation of appropriate updates to the clinical team regarding the care of patients under your administration. You will need to possess excellent communication skills, be comfortable dealing with patients and their relatives and/or carers, and be able to prioritise your work effectively. You will be joining and working closely with an innovative and supportive team of healthcare professionals who are committed to providing the best possible care for our patients.
Hours of work are negotiable but will need to be within the core opening hours of the practice, Monday to Friday, 08:00 to 18:00.
