Defined Term employees are usually hired to work in a specific department on specific long-term project work until the work is completed or to a specific end date not to exceed December 31, 2026. This is a Defined Term and fully benefited position. We have an opportunity to join the Alliance as a Care Coordinator - D-SNP in the Care Management Department. WHAT YOU'LL BE RESPONSIBLE FOR Reporting to the Medicare Care Management Manager (RN), this position: Supports the Medicare Dual Special Needs Plan (D-SNP) Care Management team with coordination of services and activities for members enrolled in D-SNP Collaborates with internal and external partners to promote efficient care coordination and support members in navigating care transitions, appointments, and referrals Contributes to the effective operation and improvement of Care Management D-SNP programs WHAT YOU'LL NEED TO BE SUCCESSFUL To read the full position description, and list of requirements click here. Knowledge of: The principles and practices of health care and health care systems The principles and practices of customer service The principles and practices of health education The diverse needs of the Medi-Cal and Medicare D-SNP populations Quickly learn, understand, and communicate the workings of the Alliance, particularly the Utilization Management and Complex Case Management, Community Care Coordination, ECM, and Member Services departments Ability to: Educate on care coordination for the D-SNP program and make presentations to individuals and groups Interpret, explain, and apply policies, procedures, and guidelines Clearly, completely, and independently document, summarize and resolve member’s concerns and inquiries, and recognize those issues requiring escalation to a higher level Organize and prioritize work, follow up as needed, meet timelines, and ensure compliance with regulatory requirements Education and Experience: High school diploma or equivalent Three years of experience in a health care setting that involved interacting with members, patients and/or providers in meeting service needs (an Associate’s degree in Health, Social Services or a related field or current certification as a Medical Assistant may substitute for one year of the required experience); or an equivalent combination of education and experience may be qualifying OTHER INFORMATION We are in a hybrid work environment, and we anticipate that the interview process will take place remotely via Microsoft Teams. While some staff may work full telecommuting schedules, attendance at quarterly company-wide events or department meetings will be expected. In-office or in-community presence may be required for some positions and is dependent on business need. Details about this can be reviewed during the interview process. The full compensation range for this position is listed by location below. The actual compensation for this role will be determined by our compensation philosophy, analysis of the selected candidate's qualifications (direct or transferable experience related to the position, education or training), as well as other factors (internal equity, market factors, and geographic location). Zone 1 (Monterey, San Benito and Santa Cruz) $26.33—$42.14 USD Zone 2 (Mariposa and Merced) $23.96—$38.35 USD OUR BENEFITS Available for all regular Alliance employees working more than 30 hours per week.
