Responsibilities
  • Coordinate patient care activities, including scheduling appointments, arranging referrals, and ensuring follow-up care
  • Maintain accurate and up-to-date patient records in electronic medical record systems
  • Communicate with patients, healthcare providers, and insurance companies to ensure seamless care coordination
  • Provide individuals with physical, emotional, and behavioral support allowing them to actively participate in activities offered while encouraging personal choices and assisting to maintain their personal safety
  • Provide transportation, in personal and company-owned vehicles, throughout the day to and from the individuals’ homes and activity locations in the community
  • Educate patients on available resources and support services
