Key Responsibilities As a Care Coordinator, you will play a critical role in leading and supporting a team of caregivers and companions, ensuring that our clients receive the highest level of care. Your responsibilities will include: * Leading and supporting a team of caregivers and companions, fostering a collaborative and compassionate environment. * Overseeing the interviewing, hiring, and onboarding process for caregivers and companions, ensuring that we maintain a team of highly qualified professionals. * Ensuring client satisfaction by maintaining high Net Promoter Scores and addressing any concerns promptly and effectively. * Providing regular, constructive feedback to caregivers and companions, helping them grow and excel in their roles. * Holding caregivers and companions accountable to high standards and company goals, ensuring consistency in care delivery. * Managing the client database, ensuring that all records are current and accurate, including care plans, demographic information, and schedules. * Overseeing staffing and scheduling to ensure that clients receive care from consistent caregivers or companions. * Maintaining compliance with Department of Health and Human Services regulations for home care. * Proactively working to increase client billable hours through effective care management. * Conducting client visits and follow-up calls to ensure ongoing satisfaction and address any changes in care needs. * Participating in the on-call rotation to provide support as needed. Why You’ll Be a Great Fit * Preferred: Bachelor’s degree in Social Work, Geriatric Care Management, or a related field. * Preferred: Two to three years of experience in case management or a related field, particularly in geriatric care. * Preferred: Two to three years of experience in managing a team, with a proven ability to provide feedback and hold team members accountable. * Demonstrated success in building strong relationships that contribute to achieving organizational and revenue goals. * Strong organizational skills with the ability to manage multiple priorities and deadlines. * Excellent communication and interpersonal skills, with a focus on active listening and effective problem-solving. * Proficiency in using online scheduling systems and maintaining accurate records. * A commitment to Golden Harmony’s Core Values of Trust, Compassion, Unity, and a Can-Do Attitude.
