Care Coordinator Salary position and you must be able to take call rotation. Reports to Director Summary: The Care Coordinator(s) are responsible for ensuring the essential functions of the office are completed daily. These include: scheduling, supporting and directing caregivers, customer service to clients, clerical tasks, and recruiting. The Care Coordinator will report directly to the Director of Operations and have significant interface with the HR Supervisor/Care Coordinator, Client Relations Supervisor/Care Coordinator, and Director. The Care Coordinators are regularly involved in situations that can be stressful and frustrating but there are also regular experiences that can be highly rewarding. Minimum Qualifications: · High School Diploma or GED · Bachelor’s degree highly preferred · Two or more years of experience in a fast-paced office environment with experience taking on-call · Ability to speak and write at a high level including medical terms · Ability to proficiently use Windows OS, Internet Browsers, Email Programs, Microsoft Office, and Adobe Reader · Ability to lift, pull and push, at least 30lbs and stand, bend, twist and sit with at least 30lbs load · Scheduling and recruiting experience strongly preferred · Experience providing personal care is required; preferred CNA. · Must demonstrate excellent oral and written communication skills and the ability to listen effectively · Must have the ability to work independently and work as a team member * Maintain confidentiality of information and meet deadlines · Must demonstrate effective interpersonal skills as well as sound judgment and good decision-making skills Essential Duties and Responsibilities: · Primarily responsible for ensuring client satisfaction and thorough communication with clients with 100% follow up o Receiving and processing various requests and schedule modifications o Sending care schedules and care plan information so that caregivers know and understand the client and expectations o Customer satisfaction follow up calls and visits · Primarily responsible for equipping and supporting caregiving staff o Completing assessments and updating care plans o Provide information, directions, paperwork, etc. as needed o Create warm, respectful, and supportive work environment for caregivers · Assist/manage resolving differences or disputes between caregivers, clients, and family · Receive initial calls with prospective clients o Compassionately listen to their needs o Collect adequate information o Provide basic information about our services o Pass on all details to appropriate staff · Assist in monitoring Caregiver performance and quality of care o Must be able to drive throughout Spartanburg and Greenville counties; daytime and during night-time hours o Build applicant files and other related administrative tasks o Assist with caregiver onboarding · Thoroughly document activities and communications in our management software · Various clerical tasks related to daily business operations and facility cleanliness · Share rotating weeks “on call” during evenings and weekends after a sufficient training period · Exercise discretion and independent judgement in the performance of the above activities · Strive to fulfill our core values and mission · Additional activities as needed Supervisory Responsibilities: · Care coordinators are responsible for the routine supervising of the caregiving staff. Dress code is business to business casual. There will be extensive use of computers, telephones, printers/copiers, common office tools, and paper files. 50% or more of a typical day will be sitting or standing at a desk. There will be occasional needs to move large quantities of paper/files or office supplies. Some travel and errands in the local area will be required. Some personal care/companionship may be required if a shift cannot be covered (if sufficiently staffed; this would be low). Hours: Care Coordinators can expect to work 9a to 5p Monday-Friday. The office is open for business 9a-5p Monday-Friday and must be sufficiently staffed during those hours. Special projects or unfilled shifts may require additional hours. Being “on call” will require work during the weekend and between 5p and 9a. Compensation: Full Time Care Coordinators receive PTO and are eligible for company benefits. Company car is available as needed for assessments, visits, supervisory needs, etc. Discretionary quarterly bonuses may be issued for excellent performance as business profitability allows.
