Day-to-day tasks include assessing care needs, developing and updating care plans, scheduling and coordinating home care visits, and ensuring that care workers have the information and guidance they need to deliver safe, person-centred support. The role involves regular communication with individuals, their families, and external professionals to monitor outcomes and resolve any issues or concerns promptly. The Care Coordinator will maintain accurate records, support compliance with relevant regulations and company policies, and contribute to continuous improvement of service quality and customer experience.Qualifications Candidates should possess skills in Care Coordination and Case Management to plan, organize, and review care packages effectively.
Candidates should possess strong Communication and Customer Service skills to interact professionally and empathetically with individuals, families, and colleagues.
Candidates should possess knowledge and experience in Home Care to understand regulatory requirements, safety standards, and best practices in community-based support.
Relevant experience in health and social care, care administration, or support services is beneficial.
Ability to work collaboratively in a multidisciplinary environment and manage competing priorities is desirable.
Proficiency with scheduling systems, electronic records, and basic IT tools is an advantage.
A relevant qualification in social care, health care, or a related field, or equivalent experience, is preferred.
