We are seeking a Care Coordinator to provide a range of coordination and support services to Cogent’s clients and customers, ensuring excellence in and quality of customer and client service delivery. Job Description We are growing and are currently seeking 1 full‑time and 1 casual team member to join our expanding team. The responsibilities include telephonic appointment booking and administrative functions, but are not limited to: Undertaking of relevant case coordination activity including liaison with all stakeholders; provision of email updates; and creation of appropriate documentation such as a preemployments assessments or other documentation as required. Completing the coordination and booking of medical appointments for a range of health services nationally. Following up with Health and Medical practices regarding results and upcoming assessments. Providing accurate, timely and concise emails or written responses in line with customer requirements and Cogent communications/service delivery guidelines. Using systems and databases for referrals and outgoing reports including an elite health booking system. A full position description is available upon request. Please email [email protected] for details. Qualifications We are looking for: Prior experience in Healthcare Bookings and Care Coordination. Hold a current and valid Working With Children Check (WWCC) Understanding of basic medical terminology. Well-developed written and verbal communication skills. General administrative support including printing, filing, photocopying, scanning, and data entry. Strong customer service focus. IT Savvy. Time management skills.
