Job Title: Care Coordinator Location: Columbus, OH Job Type: Full-time Griswold Home Care of Franklin/ Delaware counties is a well-established, respected provider of non-medical home care services. We are currently seeking a Care Coordinator to join our team. The successful candidate will be responsible for coordinating and managing all aspects of home care services for our clients. Responsibilities · Manage and Coordinate the scheduling of a caseload · Evaluate clients' needs both via the phone and in person at client’s home, facility or hospital to match the right caregiver with the client, both in skill level and personality · On boarding of new clients and caregivers. · Visit clients to ensure satisfaction · Focus on the quality of care and build warm client, family, and caregiver relationships · Provide guidance and loving support to clients, families, and caregivers to ensure an exceptional home care experience · Mandatory after hours “on call” rotation. (extra $550/week when on call) Requirements · Caring, empathetic, and compassionate individual · Effective communicator with a pleasant phone manner. · Strong verbal & written skills · Ability to build strong rapport with caregivers, clients and families · Creative problem-solving skills · Organized, detail-oriented multitasker · Good documentation, clerical and follow up skills · Computer proficiency · Experience with WellSky software is a plus.
