About the Role
As a Care Coordinator, you’ll play a pivotal role in supporting our participants to access the services and supports they need through the Commonwealth Home Support Program (CHSP). You’ll lead comprehensive assessments, develop personalised care plans, and coordinate wrap-around services that reflect each person’s individual goals and preferences.
You’ll also be responsible for supervising a team of committed Care Workers—providing day-to-day guidance, mentoring, and leadership to ensure safe, high-quality support is delivered consistently.
From the moment a participant is introduced by our Customer Service team, you will be their key connection—ensuring their journey with CRCS is warm, supported, and empowering.

The Living Well Care Coordination function is at the heart of CRCS’s commitment to ageing with dignity. We provide warm, person-first support to our participants accessing clinical and/or personal care under CHSP. Our model is rooted in trust, compassion, and meaningful connection—because we believe in care that goes beyond the checklist.
CRCS is committed to ensuring the safety and wellbeing of all young children and young people. As part of our commitment to child safeguarding, we require all employees to undergo relevant screening processes, including Working with Children Check (WWCC) and/or Working with Vulnerable People (WWVP) checks and National Police Checks.

Responsibilities

Current First Aid and CPR Certification
Certificate IV or Diploma qualification in Aged Care related field (or working towards or willing to obtain)
Current Driver’s Licence and access to a registered, comprehensively insured motor vehicle for work related purposes
A caring, respectful and empowering approach to supporting others
Strong knowledge of aged care services and care coordination practices
Excellent communication and relationship-building skills
Leadership capabilities with a supportive and collaborative mindset
Key Responsibilities
Conduct holistic, person-centred assessments and care planning for ageing people living at home
Coordinate and monitor services to ensure supports are delivered in line with participant goals and preferences
Provide leadership and daily supervision to CHSP Care Workers
Manage participant budgets and oversee service delivery within financial parameters
Conduct quality audits and contribute to continuous improvement initiatives
Lead feedback and complaints resolution with empathy and professionalism
Ensure safety and risk management practices are embedded in daily operations
