JOB OVERVIEW

We are seeking a dedicated and compassionate Care Coordinator to join our team. This position requires excellent organisational skills and the ability to communicate effectively with both patients and healthcare professionals. The ideal candidate will be proactive, detail-oriented, and committed to enhancing the quality of care provided.

PATIENT EDUCATION & HEALTH PROMOTION

  • NHS app/ online records access enablement.
  • Signposting to appropriate services.
  • Promoting and championing new & existing services.
  • Supporting the Patient Participation Group with administrative tasks when required.

QUALITY REQUIREMENTS:

  • To ensure that all patient information is managed promptly, efficiently and in a confidential manner.
  • To respond proactively to patient needs to maintain the high standard and reputation of Mann Cottage Surgery.
  • To ensure the Practice Code of Confidentiality is always adhered to.

TRAINING AND DEVELOPMENT

The Practice is committed to supporting learning and development to improve Practice performance and the post holder is expected to:

  • Actively participate in any training that the practice manager/ GP partners advise as integral to the role.
  • To seek out webinars and training courses relevant to own role and personal development, for discussion and approval by practice manager.
  • Actively participate in introducing new technology that is available to the practice.

Job Type: Part-time
Pay: £13.00 per hour

Responsibilities

ADDITIONAL ADMIN TASKS

  • Child immunisation and SCID (newborn blood spot) administration.
  • Cervical screening admin (third invitations).
  • Insurance paperwork processing – logging, assigning to correct GP for completion, speaking to third parties, ensuring patient is aware if they are to pay any fees, asking PM to issue invoices.
  • Friends and family test – issuing texts to gain patient feedback.
  • Providing cover for reception team in event of absence due to annual leave or illness.

RESPONSIBILITIES:

In line with Practice policies and protocols, to be efficient in the following:

  • The use of the Practice clinical system (SystmOne) to ensure accuracy in recording and capturing relevant patient information.
  • Effectively communicating with patients, colleagues and third parties.
