Manage and support a caseload of DDD members Conduct home visits and build strong relationships with families Coordinate Attendant Care, Habilitation, and Respite services Advocate for member needs and ensure services align with PCSPs Collaborate with DSPs, families, and support teams Monitor service utilization, compliance, and outcomes What We're Looking For We're seeking motivated professionals with a heart for service and experience in: HCBS, DDD, Case Management, or Care Coordination Group Home Management or Direct Support Professional (DSP) roles You'll thrive in this role if you: Have strong communication & relationship-building skills Are organized, detail-oriented, and solutions-driven Understand Arizona DDD systems and service delivery Are passionate about helping individuals with developmental disabilities live meaningful lives Requirements Experience in HCBS / DDD / Case Management / DSP roles Knowledge of Arizona DDD programs and services HIPAA compliance and confidentiality awareness Strong computer skills (Google Workspace, Apple preferred) Valid Level 1 Fingerprint Clearance Card Valid driver's license & reliable transportation Ability to pass APS & CPS registry checks About Endeavor HCBS At Endeavor, our mission is simple: To serve and enrich the lives of our clients, staff, and community.
