ROLE OVERVIEW

As a Care Coordinator, you will be pivotal in delivering coordinated, high-quality, person-centred support. You’ll act as the primary liaison between service users, families, internal teams, and external agencies such as CMHTs and social services, ensuring a seamless care experience that promotes autonomy, stability, and well-being.
