Premier Physician Network Quality FT / 80 hours per pay period General Summary/Responsibilities: The care coordinator-navigator responsibilities include but are not limited to outreach to Premier Health members supporting general health maintenance which will include in scheduling appointments and office visits. This role will also communicate with providers and patients for planning, address gaps in care, and communicate barriers to care. Additional responsibilities are that this role with be the lead on any projects assigned by the director of Clinical and Quality Services. Knowledge/Skills • Experience in a physician practice setting preferred. • Demonstrate accurate and timely data and computer skills. • Excellent written and verbal communication skills. • Must be self-directed with the ability to work independently. • Ability to prioritize projects, and multi-task as appropriate. • EPIC knowledge preferred. Essential Duties & Functions: • Work closely with all clinical care teams and serve as a resource. • Build a trusting, professional relationship with the patient, family, and/or caregivers. • Utilize self-management and shared decision-making tools. • Maintain accurate and timely documentation in a confidential and compliant method. • Utilize consistent documentation throughout EPIC system, following identified process. • Maintain strict confidentiality and follow HIPAA guidelines. • Knowledge of PPN systems and procedures • Ensure appropriate clinical documentation is completed/maintained. • Outreach to Premier Health members to assist with general health maintenance. • Organize and lead any projects that are assigned. • Measure project performance using appropriate systems, and tools. • Present data to leadership that is collected during projects. • Coordinate with departments/teams that may be involved with assigned projects.
