Health Insurance (covers mental health) Dental Insurance Vision Insurance Disability Insurance (STD; LTD) Paid Time Off Paid Holidays Paid Sick Leave Paid Training Employee Assistance Program Life Insurance Retirement plan (403b) As a Care Coordinator, your responsibilities will include: Provides time-limited care coordination for 12-15 identified individuals using a combination of internal utilization reviews and SEFBHN high-utilizer recommendations. Responsible for the coordination of services until the individual is adequately connected to the care that meets their needs, and progress towards the goals of Care Coordination is achieved. Ensures individuals are engaged in their current setting (e.g., crisis stabilization unit (CSU), SMHTF, homeless shelter, detoxification unit, addiction treatment facility, etc.) to facilitate a smooth transition. Ensures that an internal process is in place to provide on-call services 24 hours a day, seven days a week. Models, coaches, and supports shared decision-making in care planning and service determination with the individual and family members (where applicable) and emphasize self-management, recovery, and wellness, including transition to community-based services and/or supports. Works with SEFBHN to develop diversion strategies to prevent individuals who can be effectively treated in the community from entering SMHTFs. Requirements New Horizons will need you to have the following qualifications: Master’s degree from an accredited university or college with a major in counseling, social work, psychology, or a related human services field and two years full time or equivalent experience working with adults and children at risk Professional license highly preferred Valid and current Florida Driver's License required with no more than 6 points in the past 5 years.
