Description Care Coordinator Healthy At Home Caregivers | Full-Time Dana Point, CA | In-Office + Limited Field Work Compensation: $46,000 – $54,000 annually (DOE) Local candidates strongly encouraged Healthy At Home Caregivers is growing, and we’re hiring a Care Coordinator to support families, caregivers, and our office team in delivering non-medical in-home care. What You’ll Do Care Coordination & Assessments Manage referrals and coordinate client assessments Schedule and complete limited in-person assessments (designated field day — typically Wednesdays) Communicate clearly and compassionately with families and caregivers Ensure assessment documentation is accurate, complete, and submitted on time Coordinate clean handoffs to scheduling so care starts smoothly Service Tracking & Follow-Up Track service starting dates, renewals, and approved hours Follow up proactively so no services or hours are delayed or missed Support alignment between care delivery, documentation, and office workflows Administrative & Office Support Maintain accurate client and caregiver records Handle documentation, forms, and follow-ups with attention to detail Support office workflows during busy periods Ensure actions are documented and easy for others to follow Team & Growth Support Support leadership with coordination and documentation Identify workflow gaps and improvement opportunities Be a reliable, calm presence in a fast-moving office What Success Looks Like You’ll be successful if: Assessments are completed on time and convert into active care Clients and families feel informed, supported, and respected Approved hours are not lost due to missed follow-ups The office runs smoothly — without constant fire drills Requirements Requirements & Compliance Ability to pass a Live Scan (background check) Ability to complete a TB screening Valid driver’s license required Comfortable working in the office and attending limited field visits Use of company vehicle during work assignments We’ll Train & Support You Paid training provided Clear systems and expectations Ongoing leadership and team support Why You’ll Love This Role Purpose-driven, meaningful work Real impact in your local community Supportive, high-performing team Clear structure with room to grow A role that evolves as the company expands Because at Healthy At Home Caregivers, no one is left behind — clients or team members.
