We’re currently seeking a full-time Care Coordinator. The Care Coordinator will help coordinate service provision for providers, facilitating communication between providers, and ensuring individuals receive the appropriate care for their individual needs. Requirements A Bachelor’s degree in social work, human services, or a related field is required Experience providing care coordination services for individuals with behavioral health concerns is preferred but not required An active Iowa Bachelor’s of Social Work (BSW) license is preferred but not required Questions About the Job?
