Description

We have an exciting opportunity for a fixed term 12 month position - Care Co-ordinator - Nurse Liasion.
1 x 18.75 hours fixed term 12 months.
Care co-ordinators play an important role within a practice to proactively identify and work with patients. The Nurse Liaison role will be fundamental in supporting the Practice Nursing Team globally across Royal Primary Care. They work closely with the Practice Nursing teams to manage a caseload of patients, acting as a central point of contact to ensure appropriate support is made available to people and their carers; supporting them to understand and manage their condition and ensuring their changing needs are addressed.
Key responsibilities
Patient Assessment : Evaluating the healthcare needs of patients and ensuring they receive appropriate care.
Coordination of Care: Collaborating with various healthcare professionals to create a cohesive care plan and completing referrals as needed.
Education: Providing patients and their families with information about medical conditions, treatments, and available resources. · Advocacy: Acting as a patient advocate to ensure their needs and preferences are respected. · Follow-Up: Monitoring patient progress and adjusting care plans as necessary.
