Position Overview:  We are seeking a full-time Care Coordinator (Medical Assistant or LPN) to join our team in Honeybrook, PA. This role is ideal for a compassionate, organized, and detail-oriented healthcare professional who thrives in a dynamic and patient-focused environment. The Care Coordinator will be a crucial member of the care team, which includes a Medical Director and Nurse Practitioner, working collaboratively to ensure that patients within the senior living facility receive timely, comprehensive, and personalized care. In this role, you will act as a central point of communication between patients, families, and healthcare providers, coordinating treatment plans and ensuring that patient needs are addressed promptly. You will manage clinical documentation, keeping patient records current and reflective of their ongoing needs and treatment. As part of the care team, you will oversee medication management, schedule tests, and coordinate specialist referrals to ensure seamless care for the patients. This position offers the opportunity to develop strong relationships within the senior living facility, working closely with the internal team to provide personalized care that meets each patient's unique health needs. As the liaison between the facility staff, healthcare providers, and patients, your role will ensure that every aspect of patient care is well-coordinated. Key Responsibilities: * Documentation: Keep patient records up-to-date in the electronic health system, including ongoing care plans. * Care Coordination: Assist with managing medications, scheduling tests, and coordinating referrals, while ensuring timely follow-through on physician recommendations. * Communication: Maintain open communication with patients, families, and healthcare teams regarding patient health and treatment. * Support for Providers: Work with the team by obtaining vital signs and other relevant information for patient visits. * Telemedicine Coordination: Assist with scheduling and managing telemedicine appointments when needed. * Urgent Care Alerts: Promptly communicate any urgent health issues to the care team. Qualifications: * Certification as a Medical Assistant, LPN, or equivalent healthcare background. * Experience in geriatrics, home health, or family medicine is valued. * Technologically proficient, particularly in using electronic health records (EHR). * Excellent communication and organizational skills.
