REQUIREMENTS

  • Proven experience in home care, senior care, or complex care is essential.

  • Strong organizational skills, with the ability to manage multiple tasks effectively.
  • A nursing qualification or equivalent healthcare experience is preferred but not mandatory.

Responsibilities

THE ROLE

We are looking for a Care Co-Ordinator with experience in Complex and Paediatric Care to oversee care delivery, manage client care plans, and support our team of healthcare professionals. You will play a key role in ensuring exceptional service quality while maintaining compliance with CQC regulations.

KEY RESPONSIBILITIES

  • Oversee daily operations of the care team, ensuring compliance with company policies and procedures.
  • Develop, implement, and manage individualised care plans tailored to each client’s needs.
  • Provide leadership and support to care staff, fostering a positive and professional environment.
  • Conduct regular assessments of clients’ health and wellbeing, adjusting care plans as necessary.
  • Liaise with families, healthcare professionals, and external agencies to coordinate comprehensive care.
  • Maintain accurate and up-to-date documentation for all client care.
  • Monitor care quality through audits and feedback sessions.
  • Address concerns or complaints from clients or their families promptly and professionally.
  • Ensure CQC compliance and adherence to all regulatory requirements.
  • Participate in on-call duties on a rota basis.
