We are looking for a Care Coordinator to join our team in Derbyshire, on a full time basis. In return you will receive £22,000-£24,000 per annum (depending on qualifications and experience).
The role will include delivering care outside of your Care Coordination hours.

SKILLS AND EXPERIENCE OF OUR CARE COORDINATOR:

• Full UK driving license with access to a car is essential;
• NVQ Level 2 in Health and Social Care and looking to work towards an NVQ Level 3
• A minimum two years care experience and minimum of one year’s experience in a Care Coordinator role.
• Good people management skills
• Able to work as part of a team as well as under own initiative
• Highly organised and able to prioritise.
• Must have Good IT skills and know how to use Outlook, Word and Excel
• Good oral and written communication skills
• Knowledge of domiciliary care provision and CQC regulations is essential.
