SKILLS & EXPERIENCE

  • Demonstrated Aged Care or other clinical case management experience
  • Understanding of Aged Care funding and Support at Home changes
  • High level competency in utilising a variety of technology including mobile devices and computers
  • Ability to identify and implement risk mitigation strategies
  • High level communication, influencing, liaison and negotiation skills
  • Established problem solving and conflict management skills
  • Adaptable to change and able to multi-task and work autonomously in a fast paced environment
  • A current unrestricted Australian Driver Licence, reliable vehicle and a willingness to travel
Responsibilities

ABOUT THE ROLE

As a Care Coordinator you will oversee and enhance the delivery of our Aged Care services in New South Wales, mainly in Newcastle and Tamworth. Some of your duties will involve processing referrals, aiding clients in commencing with our services, and continuously adjusting care plans to suit evolving needs. You will coordinate home visits to ensure the needs of the people we support are met and that we are working in partnership with them and their families.
Additionally, you’ll contribute to program coordination by assessing, improving, and overseeing operational and administrative systems. You will deliver exceptional service to ensure our clients and their families feel valued and supported, while also fostering the growth of our services.
This role will support a small caseload whilst also providing relief to other Care Coordinators to facilitate leave and ensure our clients receive continuous support. The successful candidate will be driven to support growth and establish a strong reputation for LWB among our clients and the local community. This position is based in Newcastle with some travel required to clients’ homes across the region as needed, and travel to Tamworth at least annually.

KEY RESPONSIBILITIES

  • Undertake comprehensive client assessments while developing client profiles and care plans in collaboration with the client/representative and other relevant stakeholders
  • Ensure care plans are monitored and reviewed on a regular basis to align with the changing needs of clients, including regular client home visits
  • Monitoring the clinical risks and needs of a client, ensuring referral pathways are established for the safety of our clients
  • Collaborate effectively with internal stakeholders to ensure clients’ needs are met including our Rostering team and Practice Leads
  • Represent Life Without Barriers at external forums and meetings, promoting the organisation within the community
