Due to maternity leave, we are seeking a reliable and organised Care Coordinator to join our office team—this is a fixed-term role with the potential to become permanent.

THE ROLE

The Care Coordinator is a key part of our operations team, responsible for ensuring care is delivered smoothly and safely in line with each client’s care plan and preferences.

KEY RESPONSIBILITIES:

• Scheduling and coordinating home care and live-in care visits
• Ensuring continuity of care for clients through effective rota planning
• Managing incoming queries and last-minute changes to care schedules
• Supporting recruitment, onboarding, and compliance processes
• Maintaining accurate records in line with CQC requirements
• Liaising with local authorities, healthcare professionals, clients, and families
• Participating in the on-call rota (additional payment provided for on-call duties)
