JOB SUMMARY

We are seeking a dedicated and compassionate Client Care Coordinator & PSW Supervisor to join our team.
The Client Care Coordinator is responsible for managing client intake, developing care plans, and ensuring the delivery of high-quality homecare services. This role acts as a liaison between clients, families, PSWs, and other circle of care professionals.
The PSW Supervisor oversees the recruitment, performance and development of Personal Support Workers (PSWs), ensuring the delivery of safe, respectful and effective care to clients in their homes. The ideal candidate will have a strong understanding of home support and employment standards in New Brunswick.

QUALIFICATIONS:

  • Successful completion of Criminal Record Check and Department of Social Development Vulnerable Sector Check
  • Possess and maintain valid driver’s license & current vehicle insurance, with a minimum of $1 million coverage. Note the agency does recommend that employees obtain $2 million coverage
  • Must have a minimum of grade 12 education, post-secondary an asset
  • Must have strong written and oral English communication skills
  • Experience working in a team environment
  • Must possess very strong computer/software literacy skills (electronic HR records/scheduling software/Microsoft 365)
  • Demonstrated ability to solve problems and multitask
  • Proven experience in supervising staff, scheduling, and care coordination an asset
  • Red Cross CPR and First Aid certification
  • Strong leadership and communication skills and ability to train, mentor, and evaluate PSWs
  • Strong knowledge of provincial homecare regulations and documentation standards

    Pay: $17.42-$21.00 per hour

Responsibilities

RESPONSIBILITIES – CLIENT CARE:

  • Intake assessments and follow up home visits as necessary
  • Care Plan development
  • Scheduling
  • Client communication
  • Accurate Documentation
  • Timely and appropriate Coordination with PSWs, EMS, DSD, DVA and other circle of care professionals and client family caregivers
  • Address client concerns and service delivery issues
  • Maintain accurate and up to date electronic care plans and service records
  • Ensure that Payroll/Accounting Coordinator has accurate financial/billing data
  • Provide on-call rotation
  • Participate in development of strategies and opportunities to improve services and promote Home Support Services Inc.

RESPONSIBILITIES – PSW SUPERVISOR:

  • Recruit and hire PSWs as required (maintain applicant files, interview, complete reference checks, and provide orientation per the New Employee Handbook)
  • Supervision of PSWs, including addressing all issues requiring remediation or disciplinary action, including termination
  • Arrange and coordinate regular and routine staff meetings
  • Maintain accurate and up to date electronic employee HR records.
  • PSW Training and mentoring
  • Provide on call rotation and train and supervise PSW candidates for on call duty
  • Initiate a good working relationship with Social Workers and other government contacts and maintain regular flow of communication
