About The Role

As a key member of the ATSICHS Brisbane team, contributing to excellent patient care the Care Coordinator Nurse supports patients with Chronic Illness. The Care Coordinator is responsible for the provision of effective service coordination working closely with clients with the aim to achieve improved health outcomes for all Aboriginal and Torres Strait Islander people.

Duties

- Provide care coordination to assist in the provision of chronic disease management.

- Establish and maintain relationships with key external stakeholders conducive to holistic health and social support needs.

- Foster a culture of continuous improvement and innovation within the Primary Health Support Services, integrating culturally safe practices.

- Ensure effective implementation and maintenance of ATSICHS Policy and procedures in line with AGPAL Accreditation Standards.
