Description:

The Senior Care Coordinator plays a vital role in ensuring the well-being and support of clients within a care setting. This position involves overseeing care plans, coordinating services, and acting as a liaison between clients, families, and healthcare providers. The ideal candidate will have a strong background in office management and possess excellent organisational skills to effectively manage multiple responsibilities.

QUALIFICATIONS AND EXPERIENCE

  • Valid UK driving licence and daily access to a car
  • Current and ongoing right to work in the UK
  • Use of a scheduling / HR / recording system
  • Previous Domiciliary Care experience
  • Experience in a customer service setting woud be an advantage

Responsibilities:



    • Develop and implement individual care plans tailored to the needs of clients.

    • Coordinate with healthcare professionals to ensure comprehensive service delivery.
    • Maintain accurate records of client progress and care activities.
    • Communicate effectively with clients and their families regarding care options and updates.
    • Organise appointments, transportation, and other logistical needs for clients.
    • Monitor the quality of care provided and address any concerns promptly.
    • Provide training and support to junior staff members as needed.
    • Ensure compliance with relevant regulations and standards within the care sector.
