We are seeking Care Coordinator who is motivated, reliable, and passionate about aged care.
Delivering services under a range of funding sources including private pay clients, My Aged Care and NDIS, the primary responsibility is to provide quality in home care and assistance to our fast-growing client base.
The successful applicant will be working with our care team, liaising with Clients, Family Members, Staff and a wide range of Stakeholders to ensure services are provided in accordance with client’s needs, goals and preferences.
Assessment, planning, and reviewing care plans as required, services are Client focused and Consumer always directed with quality outcomes achieved using a flexible, responsive, creative and collaborative approach.
In addition to case management, this role will manage inside and outside sales enquiries, develop and maintain new and existing referral sources.
