Sacred Circle Healthcare is seeking a full-time Care Coordinator Supervisor for Wendover/Ibapah. Job Summary Responsible for overseeing Care Coordination and Community Health Workers (CHW) services, as well as supervising Care Coordinators and CHWs at Sacred Circle Healthcare’s Wendover and Ibapah locations. This individual will work closely with the Sacred Circle Healthcare Executive Director, Medical Director, Substance Use Disorder Clinic personnel, CHW leadership, and Tribal liaisons to ensure effective care coordination for CTGR patients and identified AI/AN community members.  Reports To: Care Co ordinator ManagerDepartment: Care  CoordinationBusiness Unit: S acred Circle OperationsPatient Care: Yes Worker Status: Full-ti meBenefits Eligible: Y esHours Per Week: 4 0Work Schedule: To Be Determined with the availability of 8:00 am – 6:00 pm Monday through Friday. Evening and weekend work may be required as job duties demand.  Supervisory  Responsibilities: Care Coordinators The successful candidate: * Oversee and supervise Care Coordinators and CHWs, including training, monitoring performance, and reporting. * Complete proper confidentiality releases to coordinate between the patient and community partners. * Coordinate needed services internally within SCHC departments and among the care coordination and CHW teams. * Work with agency providers to create and update care plans, including patient goals and objectives.  * Create and manage referrals, including linking patients to external providers, programs, and agencies; scheduling appointments; and assisting patients with applications as needed.  * Monitor and follow up with CTGR patients, CTGR families, assigned AI/AN patients, and identified community partners.  * Network regularly with clinicians, CHWs, and community service partners to ensure positive working relationships.  * Network with tribal partners to maintain strong and supportive partnerships.  * Maintain timely and accurate records including dates of services, appointment details, session goals, progress notes, referral outcomes, timelines, reassessments, and coordination with other programs.  * Conduct regular Care Coordinator and CHW team meetings to provide oversight, education, training, and communication of expectations, needs, and opportunities.  * Work with identified leadership to develop expectations, accountability measures, and workflow improvements; serve as a resource to Care Coordinators and CHWs and support productivity.  * Travel to SCHC clinics, community agencies, Wendover, and the Ibapah Reservation as needed. Travel is included in the salary.  * Take after-hours or weekend phone calls as necessary.  * Provide in-home support, education, care coordination, and follow-up services in collaboration with Care Coordinators, CHWs, and clinical staff.  * Maintain clear, timely documentation of all home visit activities, assessments, referrals, and outcomes in the patient’s record.  * Perform other duties as assigned.  Minimum Requirements: EDUCATION:  Associates degree or equivalent of 5 years of experience in a healthcare related field. EXPERIENCE:    CERTIFICATION/ LICENSE: Current Care Coordinator Certification/Case Management Certification or the ability to obtain equivalent certification within 6 months of hire. Other:   Preferred Requirements: EDUCATION:  EXPERIENCE:  1+ years supervisory experience in a clinic setting. CERTIFICATION/ LICENSE: Other: Experience working with the Native American Community.
