Job Overview:
We are currently seeking a dedicated Care Coordinator to join our team in providing essential support to our team and clients. The role involves coordinating care rosters, matching our professional carer’s with our client and ensuring the best match for the care provision required.
Duties:
- Liaise with clients, and families to arrange care visits that are matched to our care professionals
- Ensure that the care rosters are planned ahead by 4 weeks at a time
- Schedule and coordinate care services, including home visits, outings and appointments
- Update carer’s and clients when changes may occur to the rosters
- Maintain accurate records of client information and maintain confidentiality
- Provide guidance and support to care staff
- Respond promptly to any roster related issues or concerns raised by clients or their families
Skills:
- Excellent communication skills, both verbal and written
- Strong organisational abilities with attention to detail
- Empathy and compassion towards clients and their families
- Proficient in using office software for documentation and scheduling
- Ability to work collaboratively in a team environment
