As we grow, we are seeking a dedicated Care Coordinator to join our team and support both client care coordination and daily office operations.

POSITION SUMMARY

This hybrid role is ideal for someone who is highly organized, detail-oriented, and people-focused. The Care Coordinator will oversee client scheduling, caregiver matching, care plan management, and in-home visits, while also ensuring smooth day-to-day office operations. The role requires excellent communication skills, problem-solving abilities, and a passion for helping others.

QUALIFICATIONS

• Previous experience in care coordination, client care management, or healthcare administration preferred.
• Strong organizational and multitasking abilities.
• Proficient in Microsoft Office Suite; experience with ClearCare/WellSky or similar scheduling software is a plus.
• Excellent verbal and written communication skills.
• Compassionate, professional, and reliable with a strong attention to detail.
• Ability to handle confidential information and work under pressure.
• Valid driver’s license and reliable transportation for in-home visits.
