OVERVIEW

We are seeking a dedicated and organised Care Coordinator to join our team. The ideal candidate will play a crucial role in ensuring that clients receive the highest level of care and support. The Care Coordinator will be responsible for liaising between clients, families, and healthcare professionals to facilitate seamless care delivery.

EXPERIENCE

  • Previous office experience is essential, ideally within a healthcare or social services environment.
  • Strong organisational skills with the ability to prioritise tasks effectively.
  • Excellent verbal and written communication skills, with a focus on building rapport with clients and their families.
  • Proficiency in using office software and electronic health record systems is advantageous.
