JOB SUMMARY

We are seeking a dedicated and compassionate Care Coordinator to join our team. The Care Coordinator will play a crucial role in ensuring that clients receive the highest quality of care by effectively managing their care plans and coordinating with healthcare providers. This position requires strong organisational skills and the ability to work collaboratively within an office environment.

REQUIREMENTS

  • Must be a car driver with access to a vehicle.
  • Ideally two years experience working as a senior for a Domiciliary Care Agency
  • Previous experience in a healthcare or social services setting is preferred.
  • Strong organisational skills with the ability to manage multiple tasks effectively.
  • Excellent communication skills, both verbal and written.
  • Proficiency in using office software applications for documentation and scheduling.
  • Ability to work independently as well as part of a team.
  • A compassionate approach towards clients and their families, demonstrating empathy and understanding.
  • Familiarity with care coordination processes is advantageous but not essential.

Responsibilities
  • Develop, implement, and monitor individual care plans for clients, ensuring that all needs are met.
  • Act as the primary point of contact for clients, families, and healthcare professionals to facilitate communication and address concerns.
  • Schedule appointments and coordinate services with various healthcare providers.
  • Maintain accurate and up-to-date records of client interactions, care plans, and progress notes.
  • Conduct regular follow-ups with clients to assess their satisfaction with services received.
  • Collaborate with multidisciplinary teams to ensure comprehensive care delivery.
  • Assist in the training and support of new staff members within the office setting.
  • Participate in quality assurance activities to improve service delivery.
