Due to our continued growth, we are seeking a Care Coordinator to join our Sligo office. This is an excellent opportunity to work within a supportive team environment, coordinating care services within a specified region.

THE PERSON: ESSENTIAL REQUIREMENTS

  • Experience of working in a care or related environment
  • Qualification in Health Care or Social Care desirable but not essential
  • Customer service orientation and able to build good relationships
  • Excellent proficiency in IT and possess a good telephone manner
  • Team player with a respectful and inclusive approach
  • Organised, positive and resilient

Responsibilities
  • Communicate effectively with the HSE re homecare packages and coordinate services for clients
  • Accept, allocate and process referrals for new work in accordance with carer capacity in an area
  • Organise carer & client work schedules that maximise capacity
  • Liaise with clients and their representatives about their care and offer support when required
  • Work closely with managers, team leaders and carers to optimise care delivery
  • Monitor the delivery and quality of care; follow-up on care concerns and produce monitoring reports
  • Promote the principles of person-centred care ensuring clients are treated with dignity and respect
