We are looking for Care Coordinators to join us, so if you’re an enthusiastic and motivated individual looking be part of a team providing excellent cutting-edge urgent community care, we’d love to hear from you!

In this role, you will be the point of contact for service users, their families, and carers when information on their crisis and short-term care support is required. This will include enquiries about anticipated visits, providing updates, and listening to their concerns.
A key element of your duties will be processing referrals for reablement support and making onward referrals via the appropriate pathway for the patient, using criteria guidelines and seeking advice from relevant team members when required. You will sign-post these referrals to MDT members and other services as appropriate while monitoring, tracking, and reporting on progress within the patient pathway. This will involve escalating any issues or delays to the Operational Team Lead for appropriate action.
