As the Care Coordinator Technician, you will play a pivotal role in individuals’ lives, making a significant impact through:

  • As directed, consult with or case conference with member’s providers on progress of care plan services
  • Document updates to care plan and assessment, including barriers and services access
  • Advocate with appropriate agencies for services and assist members in the scheduling and coordination of services.
  • Conduct home visits, as appropriate, and monitor member health status, access to services and quality of received services
  • Maintain effective communication with inter-disciplinary team serving mutual members
  • Engage in case finding activities such as outreach to community organizations

Care Coordinator Technician Minimum Salary: $35,000
Care Coordinator Technician Maximum Salary: $35,000
Care Coordinator Technician Schedule: Monday - Friday 8:30-4:30

Qualifications:

  • High School Diploma
  • 1-3 years experience (preferred, not required)
  • This position requires local travel with mileage reimbursement in personal car
