Your responsibilities will encompass managing care plans, coordinating schedules, and overseeing the day-to-day operations of care delivery. As the Care Coordinator, you will play a crucial role in optimising service delivery, enhancing client satisfaction, and fostering a positive work environment for our support staff. Responsibilities Lead the coordination of care for clients Schedule and manage staff assignments to ensure consistent and efficient care delivery. Act as the primary point of contact for clients and their families, addressing inquiries and concerns promptly. Facilitate communication between clients, families, and caregivers to promote a cohesive care approach. Assist with staff training and development to ensure high care standards are met. Maintain accurate records and documentation of client care activities and incidents. Ensure Continuity of care On-call duties as part of a rota Previous experience in a care coordination role or similar position in the healthcare sector. Excellent organisational skills and attention to detail to effectively manage multiple care plans. Strong communication and interpersonal skills to build relationships with clients, families, and staff. A passion for providing high-quality care and supporting individuals in their health journey. Ability to work independently and collaboratively within a team. Proficient in using healthcare management software and other technology tools.
