- THE ROLE WILL BE WORKING FROM BOTH OUR WARWICKSHIRE OFFICE BASED JUST OUTSIDE OF STRATFORD - UPON - AVON & OUR OFFICE BASED IN KENILWORTH

    An opportunity has arisen for a Care Coordinator to join our established friendly Team, coordinating for our Warwickshire & Coventry branches . As part of this role you will be responsible for scheduling the care hours for our care workers, matching clients & Caregivers carefully to ensure we provide high quality person centred care. Ideally you will have your NVQ level 3 in Health & Social Care or working towards this.

RESPONSIBILITIES

  • The Care Coordinator will ensure that all regulatory and contractual standards are met and exceeded.
  • The Care Coordinator will provide leadership and management to an assigned team of caregivers. They will ensure all teams have the skills and competences required and will provide a focus on continuous professional development within their team.
  • They will be responsible for ensuring all current clients are properly assessed and that high quality, person-centred care is planned for and provided.
  • The Care Coordinator will ensure that Unique Senior Care is appropriately represented and working in effective partnership with key local service providers, including local NHS partnerships, Social Work departments and community contacts.
    . The Care Coordinator will act as an ambassador for the organisation.
    . Supporting the Registered Manager with writing Care plans

. Business development, outreach in the local community, building strong relationships with external stakeholders

  • Communicate effectively with caregivers, clients, families, and other parties.
  • Manage caregiver schedules including planning and manging Annual Leave and other requests for time off
  • Manage all client schedules, ensuring care plan outcomes and requests are being met
  • Match Caregivers and Clients to ensure that our clients are supported by appropriately skilled staff
  • Maintain accurate client and Caregiver records on our people management system, ensuring all discussions our recorded in the relevant notes section
  • Interview of caregivers coming through
  • Maintain compliance with all legal and company requirements at all times
