key Responsibilities

• Manage a caseload of clients with complex needs in collaboration with internal and external health and social service providers
• Develop and implement integrated care plans that coordinate services within Wabano and across partner organizations
• Facilitate case conferences with clients and their support teams to enhance communication and continuity of care
• Advocate for clients to improve their experiences navigating health and social service systems
• Provide education and coaching to clients and their supports, including chronic disease self‑management, coping strategies, communication skills, and other life skills
• Accompany clients to appointments, when needed, to ensure their voices are heard and they receive appropriate, high‑quality care
• Monitor progress and adjust care plans based on clients’ goals, needs, and strengths

• Post‑secondary education in Social Work, Nursing, Health Sciences, Public Health, or a related field
• Minimum of 3 years’ experience in a related role
• Demonstrated experience in case management and care plan coordination within a team‑based environment
• Ability to connect clients to internal and external supports and complete required follow‑up
• Strong understanding of and commitment to trauma‑informed care
• Experience working with Indigenous communities and sensitivity to Indigenous issues
• Willingness and commitment to learning about Indigenous cultures to better support clients
• Excellent communication, interpersonal, and customer service skills
• Ability to work both independently and collaboratively within a team
• Proficiency with computers and common software applications
• Clear criminal reference check for work with vulnerable populations
